Prescription Forms
As a referring professional, you show your patients and clients that you not only believe in the efficacy of massage and manual therapy for their issues, but that you care enough to be sure that they are getting the most qualified and experienced massage therapists available. Below are two options for you, both the online form, which will be emailed directly to us and returned to you, or the printable form for either your patients or your staff to print out and have you complete. Either method will ensure that your patients will receive the best care and appropriate manual therapy, by your direction. You will also receive a call from us after the initial session, ensuring that we have worked with your client, and exactly which techniques and observations we have utilized.
The therapists at Rē understand that incorporating massage and manual therapy is in conjunction with your care, it does not replace your diagnosis or other prescribed treatment. We believe in integration and complementary care, not replacing medical treatments. If a referral to another medical professional is what the client wishes, we will endeavor to have them communicate their issues with you, the referring professional.

All emails are strictly confiential, and are retained with the client's records. If you wish, you can print this out to take with you to your next doctor's appointment.
Does insurance cover Therapeutic Massage and Manual Therapy? Put simply, yes and no.
In most cases, (especially Major Medical), massage and/or manual therapies are not covered, unless performed by a Physical Therapist or Physican. But worker's compensation, or insurance situations arising after a automoblie accident may. There is specific paperwork to fill out, and appropriate guidelines and competencies that need to be met, but in those cases, we are willing to discuss options so that you can recieve the care that you need.
Also, most flex spending plans may reimburse you for your sessions at Rē!
